Membership Requirements
In order to be a ski club participant, it is an insurance company requirement that you be an ACTIVE member of USA Waterski. Go to www.usawaterski.org/renew to complete an on-line application. Additionally, if you drive your boat for any club sponsored events, you need to show USA Waterski as additional insured on your boat owners policy. Please contact Dennis Moore for more information. Membership Application
The two files below both contain the Lake Shannon Waterski Club Application - the first is Excel format, the second is a pdf. Every family that participates must complete the application and submit the signed form along with their membership fees of $50 to either Dennis Moore, Claus Schnabel, or Eric Gulbis.
The first is an Excel file and the second is a PDF file for those of you that don't have Excel.I encourage everyone to try to use the Excel version since it's not only easier but allows us to store and retrieve the data more efficiently. I've included directions below:
EXCEL USERS
1. Download the attached file. Remember what directory you are saving it to.
2. Start Excel and open the membership application Excel file. It may tell you that the file contains macros. These macros are safe and are there to simply to automate certain functions. Make sure you enable macros, otherwise the drop downs and auto-fill functions will not work.
3. IMPORTANT - The very first thing you should do is Indicate your current status. If you indicate you are a returning member,you will be shown a drop down box where you can select your name. Doing so will fill in much of the basic information for you saving you a lot of time.
4. Review both Page 1 and Page 2 (these are represented as tabs at the bottom of your Excel screen) and fill all required fields.
5. Print both Page 1 and 2. Review the Waiver and Release of Liability paragraph. Sign only if you agree with these terms.
6. Please return the completed Excel form to Dennis Moore (midwestden@aol.com) and submit the printed and signed form with payment to Claus, Eric, or Dennis.
PDF USERS
1. Open the attached PDF file. If you do not have Adobe Acrobat, you can download Adobe Acrobat Reader at no charge directly from Adobe.
2. Print the two-page form and fill in all required fields.
3. Review the Waiver and Release of Liability paragraph. Sign only if you agree with these terms.
4. Please submit the printed and signed form with payment to Claus, Eric, or Dennis.
